List sources of financial support you are receiving for this educational program, scholarships, grants, loans, etc. Please be as accurate as possible when quoting amounts.

There is an internship component involved with this support program. The applicant will be expected to help in the church in the area in which he/she is comfortable (youth fellowship, teaching Sunday School, secretarial services, janitorial, etc.), and this commitment is to be done on a regular basis. Please list below the talents and/or work experience you could apply toward fulfilling the internship. If you are currently interning, describe your service, number of hours served monthly and where service occurs. Will you continue to serve the same place if given financial consideration. If not, where?

Two letters of references are required of each applicant. One should be from a pastor in your church and one from another person who will address your character and performance qualifications. Please list the names and addresses of those references. If re-applying for funding a letter verifying service is required. No further references are needed. For a re-application, a transcript of current grades is to accompany this form.
